If you need additional space, print or photocopy as many copies of page 2 as needed. If you are charging entry fee and completing multiple forms, credit card information need only be given on one page. For your security, delete credit card information from copy of form enclosed with your wine shipment.
Use of your own form is acceptable as long as it includes all requested information. Horizontal format preferred. Completed forms may be returned by email, fax or mail. Copies of labels are requested to assure we state the name of the wine as it appears on the bottle, so please include them with the entry form, not the wine shipment. Photocopies are acceptable, as are tech sheets if they include the label image. Labels may be mailed if you are faxing or emailing the entry form if labels will not be legible when faxed or you do not have them in digital format.
Please keep a copy of the completed entry form for your records.
